Our Light The Way Program is designed to help independent individuals plan for their future as they age. It is a service that offers guidance and support to seniors who are looking to take an active role in directing their own aging journey, rather than reacting to challenges that arise with age. This program serves as a safety net to provide individuals who may live alone, who have no children or whose family
lives far away. It offers a sense of security and peace of mind, especially during difficult times or unexpected events.

- Review of Care Plan and Emergency Packet
- 3 – month virtual check in meeting
- 6 – month in person check in meeting
- 9 – month virtual check in meeting
