Lighthouse Senior Care Consultants is seeking a compassionate Aging Life Care Professional / Geriatric Care Manager to help aging adults and their families navigate complex healthcare, home care, facility transitions, long-term care needs, and quality-of-life decisions.

We welcome qualified professionals from a variety of disciplines, including Registered Nurses (RNs), Licensed Practical Nurses (LPNs), gerontologists, physical therapists, occupational therapists, speech-language pathologists, social workers, and other experienced healthcare providers.

In this role, you will use your professional judgment, assessment skills, care coordination experience, and advocacy abilities to support clients across home, healthcare, residential, and community settings. The Aging Life Care Professional / Geriatric Care Manager provides professional care management services for
adult and aging clients who need unbiased, knowledgeable, and compassionate support. Responsibilities
include, but are not limited to:

  • Conduct comprehensive client assessments and identify health, functional, cognitive, psychosocial, safety, and quality-of-life needs within professional scope of practice.
  • Develop, implement, and update individualized care plans that reflect client goals, care needs, family concerns, and available support systems.
  • Coordinate medical appointments, home care, therapy services, community resources, facility transitions, discharge planning, and other ongoing care needs.

  • Monitor client status, care plan effectiveness, medication adherence, environmental safety, and changes in health, function, or family circumstances.
  • Serve as a liaison and advocate among clients, families, physicians, healthcare providers, and other care team members.
  • Conduct routine visits in homes, facilities, hospitals, medical offices, and community settings; transport clients when needed using a reliable personal vehicle.
  • Document assessments, care plans, updates, communication, and follow-up needs accurately and promptly; attend required meetings, trainings, and participate in the rotating 24/7 on-call schedule every 6- 8 weeks.

  • Professional background in nursing, gerontology, physical therapy, occupational therapy, speech-language pathology, social work, or another related healthcare or human services discipline required.
  • Active Virginia professional license, registration, or credential in good standing when required for the applicant’s discipline or role.
  • Minimum of 3 years of relevant professional experience required.
  • Experience in geriatrics, home health, hospice, case management, discharge planning, rehabilitation, chronic disease management, dementia care, care coordination, social services, or Aging Life Care preferred.
  • Experience supporting older adults, adults with chronic illness or disability, individuals living with dementia, and families or caregivers preferred.
  • Knowledge of Medicare, Medicaid, long-term care systems, private insurance, entitlement programs, and community resources preferred.
  • Strong assessment, critical-thinking, organization, problem-solving, documentation, and communication skills.
  • Ability to work independently, manage multiple clients, prioritize competing needs, collaborate across disciplines, and navigate complex care situations.
  • Reliable vehicle, valid driver’s license, safe driving record, and willingness to travel locally throughout Northern Virginia required.
  • Proficiency with Microsoft Office, Outlook, smartphone use, web-based documentation systems, and electronic records required.
  • Ability to pass a background screening required.
  • Candidates must currently reside in Fairfax County, Arlington County, Alexandria, Loudoun County, or Prince William County.

  • Work is primarily daytime and community-based, with local travel required during work hours.
  • Attendance at bimonthly in-person staff meetings and required company trainings is expected.
  • Participation in a rotating 24/7 on-call schedule is required approximately every 6-8 weeks.
  • Applicants must be able to communicate promptly, document accurately, and manage client follow-up needs in a timely manner.

  • Ability to stand, walk, sit, bend, reach, and move safely in client homes, facilities, medical offices, and community settings.
  • Ability to lift or move up to 25-50 pounds when required by the role.
  • Ability to safely assist clients with mobility-related needs within the scope of the role, including pushing a wheelchair when appropriate.
  • Ability to remain calm, professional, positive, and solution-focused in stressful or changing situations.
